American Fidelity

American Fidelity 125 Cafeteria Plans
Section 125 Cafeteria Plans are available to eligible employees through American Fidelity. All eligible district employees are able to avoid taxation of their employee share of health insurance premiums by signing the plan document on an annual basis. The district establishes dates and times for American Fidelity to access the buildings each year to allow employees to take advantage of this benefit. AF MOBILE IS NOW AVAILABLE!!
American Fidelity is excited to announce an easier way to submit your Flexible Spending Account claims with their new mobile app, AF Mobile. AF Mobile allows you to take photos of your Health FSA, Dependent Day Care FSA, or Health Reimbursement Arrangement information and submit claims while on the go from any smart phone if you download the AF Mobile App!

Create a secured Online Service Center account,
File a FSA or HSA claim,

Delta Dental

Dental coverage is provided to the district by Delta Dental. The member’s share of the cost depends on if the care received was from a PPO, a Premier or Out-of-Network provider.

Dependent coverage is available for the spouse and unmarried children up to the end of the calendar year of the 23rd birthday. A Summary of Benefits and a Member Handbook can be found below. You can also find a link to Delta Dental above where a list of providers can be found on their website.

MedMutual Life

Life insurance for Washington Local employees is handled by MedMutual Life, a part of the Medical Mutual Company.

Death benefits as well as accidental death and dismemberment are provided for every contracted employee, both part-time and full-time. The amount of the insurance benefits provided depends on the classification of the employee.

To download a copy of your Employee Benefit Booklets you can click on the appropriate document that is listed below. There is a link to their Web site above. Also included under the documents is a copy of a Change of Beneficiary form. Should you need to make a change to your beneficiary(s) you can print off a copy of the form, complete it and forward it to the Payroll/Benefits department.

Information
Paramount Health Care is the district’s health insurance provider. This is an HMO plan with a network of medical providers. There is also an option for prescription coverage with a three-tier co-pay.

Dependents include the employee’s spouse and dependent children.  Overage dependents can also be covered if they meet the eligibility requirements by the Federal Healthcare Law up to the age of 26.

Did you know....  You can access all of your claims and healthcare information for you and your enrolled dependents by logging into your My Paramount page?  Check out the link provided above.  You can also access your Steps2Health information for our Wellness Program and start working on your progress to a healthier life.  Also listed above is a link to the Health and Wellness site online to help in your wellness education.  This can all be done from your smartphone as well!

Membership in STRS is required for all certified employees.  The current employee contribution rate is 14% of your gross salary.  The district contributes another 14% on your behalf.

Tax Deferred Annuity Plans

Washington Local Schools offers our eligible employees the opportunity to save for retirement by participating in the Washington Local School's 403B and/or 457 plan(s). You can participate in this plan by making pre-tax contributions. The district does not contribute to this benefit on behalf of the employee.

To start your contributions to a 403B and/or 457 plan, complete and return a Salary Reduction Agreement form (found below) and forward to the Payroll Department. Please note that in addition to completing and returning a Salary Reduction Form you must also establish an account with one of the approved investment providers. The District’s vision coverage is through Vision Service Plan (VSP). VSP has a network of providers with specific allowances for wellness visits, prescription glasses and contact lens.

Dependents include the employee’s spouse and unmarried children until the end of the year they turn age 23.  Dependents age 23-25 that are a full-time student may continue their coverage on your policy.  Contact the Payroll/Benefits department at the number listed below to find out how to continue their coverage.  

To check you or a dependents eligibility, claims, etc., click on the VSP link above and create a Member Login/Password and view all of your information.  You can also create a Reimbursement Claim online through your login.
